YOUTH MENTAL WELLNESS
(13-18 YRS)

Partners4Wellness uses a concierge mental wellness approach to provide services to youth aged 13-18. Youth are typically less likely than any other age group to access mental health services because of the lack of access to appropriate services, as well as the negative peer perception associated with mental illness, counseling, and therapy. Effective early management is critical to preventing the progression of mental disorders. Social isolation, anxiety, depression, and sense of loneliness can be prevented through early intervention. TRANSITIONAL AGE YOUTH
(18-24 YRS)

Partners4Wellness created Connect-OC to be a platform to connect 18-24 Transitional Age Youth (TAY) to local mental health services. Connect-OC is a network of services between mental health agencies, college/universities, Transitional Age Youth, Young Adults (YA), mental health peer leaders, advocates, clinicians and therapists. Connect-OC seeks to align efforts and increase and access of community health services available for students, their families and TAY-YA at-risks individuals. Partners4Wellness developed the Connect-OC Coalition to bring representatives from agencies who are aligned to meet the mental and behavioral health needs of college/university students and the TAY/YA population. With over 300 local participants, the Coalition meets monthly to share resources and enhance services.
