Who We Are

After more than 65 years leading efforts in Orange County to reduce alcohol, tobacco, and drug use among our County’s youth, Partners4Wellness is now taking a more holistic approach to well-being and the people we serve. Starting in 2024, Partners4Wellness has grown its work to address the public health issues facing Orange County residents today to help build healthier communities.

Building on our deep history and understanding of Orange County communities, we have begun a broad new initiative to connect residents to proven services and programs offered by the Orange County Health Care Agency to help them improve and maintain their health. These services range from helping people manage chronic diseases such as diabetes or hypertension, access vaccines to prevent illnesses such as COVID-19, flu and RSV, maintain a healthy pregnancy and plan a healthier lifestyle including nutrition, physical activity and preparing for your tobacco quit journey.

Through education, outreach efforts and community engagement, we aim to give Orange County residents the right tools and resources to achieve their health and well-being goals. With a demonstrated commitment to improving public health rooted in equity and compassion, Partners4Wellness is ready to be your partner in health.

Focus Areas

The following areas have been identified as having the greatest potential to impact our County’s public health. Within each of these areas, we aim to raise awareness about programs and tools that can help community members improve their health outcomes and well-being.

What We Do

Partners4Wellness has created a wide-ranging outreach strategy that emphasizes equity and inclusivity to ensure we reach all Orange County community members. Our public health work spans the following steps, all of which can be accessed by calling or visiting.

Our field teams visit individual’s homes to share information about OC Public Health Services (PHS) programs and other public health initiatives to expand awareness of available health resources. Our teams are trained to distribute health education materials, conduct assessments and provide links to services with respect and compassion.

With a long history in chronic disease and addiction prevention, Partners4Wellness is adept in communicating with community members to understand their health care needs and barriers they face to improving their health. Our team members know how to prioritize the health and social needs of a family or individual, and refer them to available resources such as organizations, facilities and programs in the community.

Linkages-to-Care/Services:

Linkages-to-Care/Services:

This area covers the comprehensive process of connecting individuals or families with the right services and resources to address their specific needs or barriers to care identified in a health assessment. During this process, Partners4Wellness may be directly involved in supporting the family or individual to access a service or resource and may include providing translation services during an appointment, coordinating transportation, and/or assisting with applications for health and medical coverage. The goal of Linkage to Services is to provide a total approach that addresses all aspects of a person’s well-being, ensuring they receive comprehensive support that reaches beyond health care to include all parts of their lives.
