Partners4Wellness, a leading nonprofit serving Orange County since 1957, is seeking compassionate and dedicated case managers/counselors for our DUI and MAT program in Irvine. This program provides high-quality, in-person DUI and MAT education and intervention services to nearly 400 clients, supporting them on their journey toward healthier, safer lifestyles. We’re looking for motivated professionals passionate about helping others, guiding clients through the DUI process, and fostering positive behavioral change.

Summary of Position

The case manager/counselor, under the direction of the program manager, executes day-to-day case management duties and responsibilities as described below.

Essential Duties and Responsibilities

  • Assess and schedule activities that ensure the participant stays current with payments and activities.
  • Perform core functions of case management as defined by the agency.
  • Assess participants needs and possess skills to refer clients for ancillary services as needed.
  • Discuss and encourage participant attendance in educational sessions and group sessions.
  • Capability to collect program fees and log payments according to agency requirements.
  • Experience in facilitating small group sessions and working in a team structure.
  • Maintain complete accuracy in participant files.

Qualifications & Experience

Experience providing case management/counseling services. Education and professional background in human services, substance use disorders, DUI education or health care disciplines. Minimum of two years of previous case management, counseling, or professional academic experience preferred. Knowledge and experience in the SUD field and familiarity with Title 9 and MAT services a plus.

In this role, you’ll plan and lead prevention and education outreach, teaching essential life-skills and resilience strategies. You’ll work directly with schools, juvenile hall, probation, and other youth-serving organizations to provide mentorship, coaching, and support. Essential Duties and Responsibilities

  • Program Implementation – Deliver engaging life-skills and resilience training workshops for transitional-age youth (TAY) ages 16-25 in schools, juvenile hall, and youth-serving organizations.
  • Outreach & Engagement – Conduct outreach to identify and engage at-risk youth, build relationships with youth, teachers, community partners to promote program participation.
  • Individual Support & Coaching – Provide guidance, mentorship, and support to TAY participants, helping them develop coping strategies, problem-solving skills, and goal-setting techniques.
  • Collaboration & Partnerships – Work closely with schools, probation, juvenile justice programs, shelters, and other youth-serving organizations to coordinate services and referrals.
  • Resource Navigation – Connect youth to mental health services, life-skills, substance use prevention programs, housing assistance, job training, and other essential support systems.
  • Workshop Development – Assist in designing and adapting curriculum to meet the unique needs of diverse TAY populations, incorporating trauma-informed and culturally responsive approaches.

Qualifications & Experience

The ideal candidate will have a bachelor’s degree in a health-related field and experience working with transitional age youth (TAY) ages 16-25 in life-skills training, emotional regulation, and pro-social development. Strong engagement, public speaking, mentorship, and leadership skills are essential, with experience in trauma-informed care, mental health, or substance use prevention highly desirable; bilingual proficiency is a plus. Recent graduates of these fields are encouraged to apply. Prior direct experience is helpful but not required.

Program Manager

Partners4Wellness, a leading nonprofit serving Orange County since 1957, is looking for a passionate and dedicated Program Manger to lead a team of five outreach coordinators working with at-risk Transitional Age Youth (16-25 years old). You’ll lead the prevention and education outreach team, teaching essential life-skills and resilience strategies. You will partner with juvenile hall, probation, and other youth-serving organizations to provide life-skills training, mentorship, and support. The ideal candidates will possess the following skills.

Essential Duties and Responsibilities

  • Program Leadership & Oversight – Provide strategic direction and leadership for life-skills and resilience training programs, ensuring effective implementation and team management and oversight.
  • Team Supervision & Development – Manage, train, and supervise five program staff and interns, fostering a collaborative, and supportive work environment that promotes professional development.
  • Program Growth & Sustainability – Identify opportunities for program expansion, funding, and partnerships while ensuring program sustainability through grant writing, reporting, and evaluation.
  • Curriculum & Training Development – Lead the design and adaptation of evidence-based curricula and workshops tailored to the unique needs of at-risk transitional-age youth (TAY), incorporating life-skills, trauma-informed and culturally responsive approaches.
  • Advocacy & Community Engagement – Represent the organization at community meetings, conferences, and advocacy efforts to enhance visibility and support for at-risk youth programs.

Qualifications & Experience

The ideal candidate will have a master’s degree, preferably in Public Health (MPH), Social Work (MSW), or a related health field, with demonstrated experience in program management, leadership, and working with at-risk transitional-age youth (TAY) ages 16-25. This role requires strong skills in team management, strategic planning, and collaboration to effectively oversee program operations, staff, and service delivery. Candidates should have expertise in life-skills training, emotional regulation, and pro-social development, along with experience in trauma-informed care, mental health, or substance use prevention. Exceptional leadership, public speaking, and mentorship abilities are essential, as well as the capacity to build and maintain partnerships with schools, probation, shelters, and other youth-serving organizations. Bilingual proficiency is a plus.
